This is a written assignment not to exceed 5 pages in length but must be at least 2 pages.  It consists of two parts.  Yes/No answers are not sufficient. What diagnostic screening tool would you use in assessing this patient for opioid use disorder?  Remember we are looking for Diagnostic Tool that would help us come to the conclusion of OUD not Withdrawal Tool.  3.). Attach diagnostic screening tool (not just the DSM) used and fill it out as best you can for this patient.  If unable to answer all questions, simply write "unable to assess".   4.). What Labs would you order and why? 5.). Where would you say this patient is regarding readiness for change?   6.)  What are possible additional Psychiatric Diagnoses other than OUD and why?  State other possible psychiatric diagnoses or dual diagnoses only here, even though it can include more than that.   7.).  What Physical Concurrent Conditions would you assess for, and how would you assess them?  State other possible physiologic considerations only here in light of the suspected DSM diagnosis (understanding that it can include more than that).   8.). Where would you rank this patient in terms of suicide risk (low, moderate or high)?  Why?   9.). Level of Care Determination:  What care level would you consider ideal for this patient and why? 10.). What pharmacotherapy would you consider or recommend for the patient after acute detox needs are met, if any?   11.). What social services would you consider for this patient and why?
